Synopsis

Root Cause Analysis (“RCA”) has often been used as a problem-solving technique to identify the underlying causes of operational risk-related incidents or events. Following the definition of operational risk, the root cause categories typically centre around four areas, namely (1) People, (2) Process, (3) System, and (4) External Factors. While the root cause(s) for Process, System, and External Factors are relatively well developed, the People root cause has not really been fully analysed, leading to the recurrence of risk incidents or events. This webinar explores how the People root cause can be further examined to uncover the reasons behind undesirable behaviours that led to those risk incidents or events. This additional analysis enables a more holistic and complete root cause analysis. Conducting behavioural root cause analysis also offers insights into underlying culture issues that should be addressed to ensure that remediation actions result in more sustained outcomes.

Partner, PwC Singapore


Alywin is a partner of PwC with professional and commercial experience in assurance and advisory services, including in People & Organisation and change management projects, as well as a wide range of Governance, Risk Management, Internal Audit and Enterprise Risk Management engagements. As part his career history, Alywin assumed a number of senior executive roles, such as Asia Head of Internal for a UK insurer, Asia Pacific Chief Risk officer for a UK insurer, and Acting Head of Internal Audit for a Singapore listed company. He is currently a board member for Singapore Business Advisors & Consultants Council Limited. More recently, Alywin has been actively engaged on the topic of Culture and Conduct. He has acquired cumulative knowledge and experience as a domain expert in designing culture and conduct solutions. He is a lead advisor on assignments on organisation’s corporate culture, risk culture, compliance culture and ethics culture. He has conducted numerous trainings on auditing risk culture and conducting governance and risk culture audits. More recently, Alywin has advised clients on the design and implementation of culture and conduct related framework and tools, as well as methodology and approaches to auditing culture and conduct. Alywin is also currently assisting an industry Conduct & Culture Steering Group in developing industry practice notes to promote good culture practices.
